Use this form to report any work-related injuries, illnesses, health issues, or near-miss incidents.
Ensure the form is filled out promptly after the incident, or ask someone to complete it for you if necessary.
Once submitted, a copy will be sent to your line manager, who will then conduct an investigation and complete the OHS1b form.
